Population Health Management (PHM) Obstacles & Automated Interventions

Under value-based care models, keeping patients engaged and adherent to care plans outside of an organization’s four walls is becoming just as important as the care delivered inside.

However, even with tremendous EHR technology, helping patients overcome care obstacles upon returning home remains a challenge.

Many providers, have not yet found an answer to adequately manage patients between care encounters. To determine how healthcare providers feel about their readiness to manage a new population health model, InteliChart conducted a study, which included more than 800 healthcare professionals, and discovered the following results:

  • 90% of survey participants believe they are up-to-date on all MIPS and MACRA legislation as it pertains to them; however,
  • 70% do not have a technology solution that allows automated patient interventions across patient populations; and
  • 20% of healthcare professionals feel unsure about their current technologies capabilities to manage patient population health.

New data-driven care approaches emerging

Fortunately, many providers are discovering tools that work in harmony with their EHR as well as other clinical and financial information systems to deliver reliable, timely insight into patients’ health status and trends. These tools are connected to internal systems as well as robust databases that contain demographic, physical environment and lifestyle data that help providers predict patient behaviors and automatically intervene before their health status declines or adverse event occurs.

Highly targeted patient outreach is based on the patient’s preferred communication mode, including text message, mobile app notification, phone call, or emails. As such, providers are empowered to take a proactive instead of reactive view of their population health management workflows thanks to connected, highly intelligent technologies offering insight, recommendations and engagement-driven outreach communication. The reimagined operations allow clinicians to efficiently manage lower-risk, lower complexity patients and devote more valued time to the highest-need, highest-risk patients.

This realigned care management paradigm is essential for success under value-based care models. In this new world, healthier, engaged patients outside the four walls translates to less spending, higher quality and greater financial returns.
